Member Support &amp; Care Coordination<ul><li>
Collaborate with a multidisciplinary care team&mdash;including counselors, prescribers, and other cross-functional team members&mdash;to ensure the delivery of high-quality, member-centered care. This includes participation in regular interdisciplinary case conference meetings to drive care team decision making.</li><li>
Build trusted relationships with members to drive change and assist members in achieving their recovery goals through empathy, motivational interviewing, and the use of lived experience (where applicable).</li><li>
Assist members in identifying personal and treatment-related goals, providing individual coaching, coping skill support, and engagement in recovery activities.</li><li>
Support unengaged members to reengage in treatment services through intensive outreach interventions.</li><li>
Support new members by introducing them to the Groups care model and supporting them through the transition to care through outreach and engagement efforts.</li><li>
Provide in-person, virtual, and telephonic support based on member and regional needs.</li><li>
Maintain a shared caseload, collaborating with the care team to ensure coordinated service delivery and removal of barriers to care.</li><li>
Support the completion of care pathways for members who are in special populations (i.e. members at the highest risk of disengagement or a poor outcome).</li><li>
Link members to healthcare, behavioral health, social service, and community-based resources; follow up to confirm needs are met.</li><li>
Assist members in navigating practical systems such as insurance, pharmacy, housing, and transportation, empowering them to advocate for their own needs.</li><li>
Manage referrals, releases of information (ROIs), and other documentation to facilitate smooth transitions of care.</li><li>
Serve as a community connector and advocate, promoting recovery-positive language, reducing stigma, and fostering inclusion within community networks. Become an expert in local community resources and recovery networks.</li></ul> 
Administrative &amp; Operational Support<ul><li>
Deliver outstanding customer service to members, visitors, and community partners.</li><li>
Maintain accurate, timely, and compliant documentation in the electronic health record (EHR).</li><li>
Support daily operations, including group facilitation logistics, scheduling assistance, office maintenance, and safety standards.</li><li>
Assist clinicians, providers, and members with treatment-related tasks, including prescription management, toxicology submissions, prescription coordination, appointment scheduling, and responding to member inquiries via various communication channels (phone, SMS, email, chat, etc.).</li><li>
Participate in weekly interdisciplinary team meetings and ongoing continuing education as required.</li></ul><div>

Knowledge of recovery-oriented care, harm reduction, motivational interviewing, and trauma-informed practices.</li><li>
Ability to identify and navigate and connect members to appropriate community resources and support systems.</li><li>
Excellent organizational and time-management and problem-solving skills, with the ability to handle multiple priorities.</li><li>
Ability to maintain confidentiality, professional boundaries, and a high standard of ethics.</li><li>
Proficiency in G-Suite, email, electronic medical records, and other digital tools.</li><li>
Strong problem-solving skills and adaptability in a team-based environment.</li><li>
Commitment to Groups&rsquo; mission and values, including member-centered care and recovery support.</li><li>
Ability to work evenings or flexible hours as needed.</li></ul>
Qualifications:
<ul><li>Salary range: $43,000-$50,000/ year *Based on experience, and education 
Schedule:</li><li>
High school diploma required; Associate&rsquo;s degree in Social Services, Human Services, Psychology, or related field preferred.</li></ul><ul><li>
Peer Support Specialist / Peer Recovery Coach certification may be preferred or required in select programs or states, if applicable.</li><li>
Minimum of two (2) years&rsquo; experience in case management, care coordination, peer support, or a related field preferred; and experience working with individuals with substance use or mental health challenges preferred.</li><li>
Experience using Electronic Health Records (EHR).</li><li>
Reliable internet access (50M download / 10M upload) and strong WiFi if working remote.</li><li>
Must meet state-specific background screening and pre-employment requirements (which may include fingerprinting, drug testing, CPR/First Aid, or health screening).</li><li>
Valid driver&rsquo;s license and access to a vehicle (if role includes community travel).</li></ul></div>
