SUMMARY</b></p><p>
Under supervision of the Program Director, the Case Manager is responsible for developing, implementing, and monitoring Life Plans for the people we support in one of our residential settings. This role ensures that services align with regulatory requirements, agency standards, and the personal goals of each individual. The Case Manager provides advocacy, coordination of services, and family engagement, while promoting dignity, independence, and quality of life for the people we support.</p><p><b>
ESSENTIAL DUTIES AND RESPONSIBILITIES</b></p><p>
The duties and responsibilities of the Case Manager will include, but are not limited to the following:</p><ul><li>
Provides social services to each person we support working toward maximizing their social functioning, enhancing coping capacity of their family, asserting and safeguarding their human and civil rights, and fostering their human dignity and personal worth.</li><li>
Develops, monitors, and updates Individualized Service Plans in accordance with regulatory and agency policies.</li><li>
Participates in and chairs interdisciplinary team meetings; documents, interprets, and follows up on all recommendations.</li><li>
Collaborates with residential staff, clinical staff, and service providers to ensure coordinated support.</li><li>
Identifies individual aptitudes, interests, and preferences and develops goals and activities that reflect person-centered planning.</li><li>
Documents functional assessments and reviews plans of care as needed.</li><li>
Provides supervision, training, and guidance to Residence Program Specialists on Life Plan implementation, data collection, and best practices.</li><li>
Maintains regular and meaningful contact with families, guardians, and community stakeholders.</li><li>
Advocates for the rights of the people we support and ensures benefits and entitlements are applied for in a timely manner.</li><li>
Coordinates admissions, including referral materials, family contacts, and arrangements for day programming.</li><li>
Serves as liaison to day programs, clinics, service coordination, and other external partners.</li><li>
Provides support in activities of daily living and community outings as needed.</li><li>
Ensures adaptive equipment is available, maintained, and used appropriately.</li><li>

Founded by the New York City Department for the Aging (DFTA) and the New York State Office for the Aging (SOFA), LSNC Aging Services provide comprehensive support to over 800 active members, fostering health, engagement, and social connection. Older adults (ages 60+) benefit from daily nutritious hot lunches, wellness programs promoting physical and mental health, educational and arts programming to encourage lifelong learning and social services. Naturally Occurring Retirement Community (NORC) or LSNC NORC specifically serves residents aged 60+ living in the Amsterdam Houses and Amsterdam Addition&mdash;a NYC Housing Authority (NYCHA) development. NORC enhances aging in place by offering social services (case management, benefits assistance, and advocacy), health promotion (collaboration with onsite nurses, wellness screenings, and health education), recreational, cultural, and social opportunities to foster community engagement and reduce isolation.</div></div><p>

We are actively seeking a highly motivated, innovative and experienced individual to join our team as the TGNB Case Manager Coordinator. Compensation will be commensurate with experience.</p><p><b>
Position Summary:</b></p><p>
This position is responsible for obtaining and logging case management (CM) assessments/reassessments and HIV Risk assessments for a targeted cohort of Amida Care&rsquo;s HIV-Negative, Transgender and Non-binary (TGNB) membership. This position will also be responsible for conducting essential case management responsibilities for this portion of the membership, including TGNB health navigation, HIV prevention health education, and referrals to community partners to address essential social determinants of health. Additionally, this position will work with external case management agencies and care teams to collaborate on case management needs and to obtain and record necessary documentation. and progress notes received from external case management agencies, by educating sites and collecting documentation.</p><p><b>
Responsibilities:</b></p><ul><li>
Conduct telephonic reassessments and HIV risk assessments every 180 days for assigned caseload.</li><li>
Refer members to the appropriate case management agency after assessment completion, if requested by member.</li><li>
Perform monthly generation and on-going review of case management site rosters and identify members who are actively receiving case management.</li><li>
Obtain comprehensive psychosocial case management assessments/progress notes for those active members assigned to a case management agency.</li><li>
Document elements of the assessment, including the needs identified and member&rsquo;s input.</li><li>
Review and address Gaps in Care with members and providers as applicable.</li><li>
Refer members deemed unreachable to the Retention in Care Unit (RICU) and external agencies as requested through Unite Us and perform referral follow-up.</li><li>
Educate members and providers regarding Plan benefits, policies, and support services available to members.</li><li>
Provide direct individual interventions including elements of case management and advocacy, referral and follow-up meetings in-person or by telephone.</li><li>
Assist in strengthening transgender programming and HIV prevention programming across all Amida Care departments.</li><li>
Participate in meetings with the Gender Identity Support Team (GIST) and the HIV Prevention Program and assist with related projects as directed.</li><li>
Assist members with linkage to appropriate TGNB-competent and GAS-specializing providers.</li><li>
Assist with responding to GAS Health navigation requests as needed.</li><li>
Collaborate with external case managers and care teams to ensure that identified needs are being met.</li><li>
Assist with the addressing of concerns or issues with CM agencies.</li><li>

  Under supervision of the 18 JDC FINS Director, the purpose of the position is to conduct conferences and make recommendations to the Court, establish FINS/IFSP (Informal Family Service Plan) agreements, and in general, accomplish the activities enumerated in the Louisiana Children's Code Title VII.

 <p>
  This position is characterized by the performance of a variety of complex administrative, case management, and legal duties. Incumbent may be responsible for on-going or re-occurring cases.
 </p>
 <p>
  Incumbent must possess knowledge of the Children's Code and available social services.
