<p><b>Care Manager MSW</b></p>
<p>The Clinical Social Worker partners with patients, families and the health care team to address and advocate for patients' social and emotional needs. The clinical social worker is responsible for providing a full range of social work services including but not limited to psychosocial assessment, treatment planning, therapeutic interventions, discharge planning, crisis intervention, and resource referral. About PACT:</b></p><p>
The Program for Assertive Community Treatment (PACT) is a nationally recognized, evidence-based approach to mental health treatment which utilizes a highly collaborative model to positively impact the lives of individuals challenged by the most severe and persistent mental illnesses.</p><p>
The DESC PACT team is centered in the concept that recovery is more successful when all providers work closely together to provide integrated support services for all aspects of participant&rsquo;s lives, including medication, therapy, social support, employment and housing.</p><p>
Team scheduling requirements reflect this commitment to provide participants with intensive wrap-around, 24/7 services at their homes and in a variety of community settings. Team members provide rotating coverage for 12 hours per day Monday to Friday and 8 hours per day on weekends &amp; holidays. In addition, all team members participate in on-call rotations to provide 24-hour crisis coverage.</p><p></p><p><b><br>
MAJOR JOB RESPONSIBILITIES:</b></p><ul><li>
Provide ongoing assessment of participants&rsquo; mental illness symptoms and make appropriate changes in treatment plans as soon as they are needed.</li><li>
Provide psycho-education to enable participants to identify their mental illness symptoms and integrate CD services as appropriate.</li><li>
Promote participants&rsquo; personal growth and development by assisting participants to adapt to and cope with internal and external stressors.</li><li>
Provide individualized assessments, counseling, supportive therapy, coaching and/or side-by-side support to clients to assist their development in the areas of activities of daily living, social &amp; interpersonal relationships, general time management and basic employment skills.</li><li>
Plan, structure &amp; support community-based activities such as obtaining and setting up housing, grocery shopping, nutrition assistance, house cleaning and other household activities, money management, social activities, and vocational support.</li><li>
Coordinate with outside service providers including inpatient and outpatient treatment services, social security, veterans&rsquo; benefits, legal and advocacy services, and personal primary care medical and dental providers.</li><li>
Provide practical help and supports, advocacy, coordination, side-by-side individualized support, problem-solving, direct assistance, training, and supervision to help participants obtain the necessities of daily living including medical and dental health care; legal and advocacy services; financial support such as entitlements or housing subsidies; money management services (e.g. payeeships); and transportation.</li></ul><p><b>
SPECIALTY AREA:</b></p><ul><li>
Provide direct clinical services including individual supportive therapy and psychotherapy to participants on an individual, group, and family basis in the office and in community settings.</li><li>
Utilize a stage-based treatment model that is non-confrontational, considers interactions of mental illness and substance abuse, and has participant-determined goals. Be comfortable with Harm Reduction approaches.</li><li>
Assess referred clients for PACT services and complete PACT intakes, including exploring diagnostic criteria for psychotic disorders.</li></ul><p><b>
LIVING CONDITIONS:</b></p><p>
Support your clients with achieving and maintaining healthy living conditions. This can include but is not limited to attending care conferences related to living conditions, outreaching and supporting clients in their residential units with tools and skills to maintain their units, coordinating with housing staff, participating in cleaning out clients&rsquo; units, and documenting barriers to maintaining healthy living conditions.</p><p><b>
OTHER:</b></p><ul><li>
Participate in daily team meetings, psychiatric consultations, clinical supervision, program meetings and in-service trainings; participate in clinical reviews and case conferences.</li><li>
Comply with the agency's clinical accountability policies and procedures; maintain current, timely and complete clinical records; participate in quality assurance reviews when assigned.</li><li>

Requirements:

MINIMUM QUALIFICATIONS:</b></p><ul><li>
Eligible for a Licensed AAC credential or any other superseding credential that meets RCW 71.05.020 requirements to act as a Mental Health Professional whose scope of practice includes independently conducting mental health assessments and making mental health diagnoses.</li><li>
Master's degree in social work, psychology or other relevant behavioral science or Bachelor of Nursing degree with specialty in mental health.</li><li>
Ability to drive an agency or personal vehicle to conduct agency-related business. A current Washington State driver's license and insurable driving record are required.</li><li>
Interest in working with clients who are difficult to engage and maintain in traditional mental health/substance use disorder programs.</li><li>
Interest or experience working with adults who are experiencing or who have experienced homelessness, have a mental illness and/or co-occurring substance use disorders, and who are involved in the criminal legal system.???</li><li>
Good oral and written communication skills including the ability to communicate and work effectively with participants and staff from various backgrounds.</li><li>

</p><p><b>RESPONSIBILITIES</b></p><ul><li>
Serve as Primary Case Manager for assigned caseload, including advocating for and coordinating client needs and services; ensuring Service Plans, Crisis Plans, and other documentation are completed on time and kept up-to-date, as applicable; and providing consistent contacts, skills training, and care coordination to work towards goals of the client, program, and contract
</li><li>Coordinate with relevant internal and external departments to coordinate client referrals, ensure timely intake into program, and develop individualized plans
</li><li>Collaborate with care team to ensure all clients access needed treatment, support, case management, and appointments, including transportation to external providers
</li><li>Outreach to client in their homes or where they are staying in the community to assess living situation and stability, and provide treatment interventions when necessary
</li><li>Ensure all interactions represent the five principles of trauma-informed care, including safety, choice, trustworthiness, collaboration, and empowerment
</li><li>Coordinate access to medications for all clients through trained staff ensuring the seven rights of medication use are followed: right person, right medication, right time, right dose, right route, right reason (including refusal), and right documentation according to agency provided medication training
</li><li>Complete daily documentation in electronic medical record on client interactions for billable and non-billable activity, medications dispensed where applicable and assigned, and other required documentation, ensuring an interaction and subsequent documentation are completed for all assigned caseload at least twice monthly
</li><li>Practice good financial stewardship of agency and contract resources, including how time is being spent, supplies are used, property is being maintained, etc.
</li><li>Monitor spending of Petty Cash and other agency money to ensure accountability and appropriate use
</li><li>Meet identified key performance indicators and metrics for the position as identified for the program, division, and agency
</li><li>Participate in discharge planning and written summaries of treatment as needed for client referral and releases of information
</li><li>Provide immediate crisis intervention and resolution as needed
</li>Always maintain client and staff confidentiality<br>

<b>The FHARPS Case Manager works with care team to ensure high standards of care are provided within the program, including but not limited to:
</b></p><ul><li>Providing appropriate levels of treatment, skills training, and assistance are provided to clients in managing mental health symptoms and developing skills and insight to manage symptoms
</li><li>Partnering as a team to deliver current programming and fill vacant position duties
</li><li>Working collaboratively with larger outpatient team of providers to support client in actively working towards treatment goals
</li><li>Coordinating care, such as scheduling, attending, and/or arranging transportation for appointments and other case management needs
</li><li>Maintaining and documenting in client charts and program trackers following agency, county, state, and federal guidelines.</li><li>
Providing and receiving input as part of client&rsquo;s care team on service plan and progress, and collaborating with other providers such as primary care provider, housing staff for entering/maintaining/exiting housing, etc.
</li><li>Facilitating regular group and individual activities, encounters, and specialized client events, such as outings and holiday gatherings
</li><li>Providing After Hours crisis intervention with enrolled clients, when assigned:
<ul><li>Answering all incoming calls and making contact within 2 hours of first call
</li><li>Provide crisis response for clients referred by Crisis Connections
</li><li>Complete assessments, such as mental status, safety, danger to self and others, etc, as appropriate for the situation
</li><li>Coordinate with other professionals, including care team, emergency room, social workers, police, family, etc. to determine least restrictive resolution
</li><li>Provide all clinical information regarding the crisis to treatment team by 9am on the next working day and complete documentation per required timelines

QUALIFICATIONS
</b></p><ul><li>Bachelor&rsquo;s degree in social science or other related field, may consider candidates who have 2+ years of relevant experience serving adults with severe and persistent mental illness and/or in diversion services in lieu of degree. For FHARPS program, credentialing as a Certified Peer Counselor is acceptable and preferred.</li><li>
Apply for Agency Affiliated Counselor credential with Washington Department of Health within 30 days of hire, if not otherwise licensed, and obtain and maintain registration and/or licensure.</li><li>
Successfully pass criminal background check before hire date.</li><li>
Obtain/maintain valid First Aid/CPR card or certificate within 30 days of hire.</li><li>
Hold a valid license for 1 year with clean driving record to use agency vehicles.</li><li>
Have legible handwriting and ability to do simple mathematics.</li><li>
Able to navigate and comprehend charts in electronic medical record.</li><li>
Able to work independently and within a team.</li><li>
Able to communicate information and ideas so others will understand. Able to exchange accurate information in these situations.</li><li>
Able to perform duties outlined above within reasonable accommodation.</li></ul></div>
