Job Description
Seeking a Nurse practitioner with cardiology experience preferably in the heart failure space to manage clients in an outpatient virtual transition of care heart failure clinic. Patients will be virtually managed for 30 to 45 days in collaboration with their outpatient primary care and cardiology providers. There will be a heavy emphasis on the management of volume status, and the titration of guideline directed medical therapy. The ability to work with an interdisciplinary team and communicate patients' needs will be paramount.
Key Responsibilities
1. Clinical Care<ul><li>
Virtually perform comprehensive history and focused physical examinations for patients with acute and chronic heart failure.</li><li>
Diagnose, evaluate, and manage heart failure in accordance with current clinical guidelines (e.g., ACC/AHA, HFSA).</li><li>
Initiate, titrate, and monitor guideline-directed medical therapy (GDMT), including beta-blockers, ACEi/ARB/ARNI, SGLT2 inhibitors, mineralocorticoid receptor antagonists (MRA), vasodilators, and other evidence-based therapies.</li><li>
Order and interpret diagnostic tests including laboratory studies, ECGs, echocardiograms, and imaging as clinically indicated.</li><li>
Assess and manage volume overload using appropriate diuretic strategies including loop diuretics, thiazide diuretics, and carbonic anhydrase inhibitors.</li><li>
Refer patients to system-based transition of care clinics for IV diuresis or in-person evaluation as appropriate, ensuring a warm handoff to clinicians assuming care.</li><li>
Review remote patient monitoring data and respond to changes in the patient&rsquo;s condition.</li></ul> 
2. Care Coordination &amp; Team Collaboration<ul><li>
Collaborate closely with cardiologists, primary care providers, electrophysiology, advanced heart failure/transplant teams, pharmacy, social work, and case management.</li><li>

Identify social determinants of health that impact self-management and coordinate interventions with the multidisciplinary team.</li><li>
Facilitate seamless transitions of care from inpatient to outpatient settings to reduce thirty-day readmission rates.</li><li>
Participate in case reviews to identify root causes of readmissions and develop team-based strategies for improvement.</li></ul>

Key Responsibilities
</div><ul><li>Legal Publications Planning &amp; Development<ul>
<li>Partner with the CLE department to identify high-value programs and convert live program materials into polished, saleable coursebooks on a predictable monthly schedule.</li>
<li>Actively seek new publication opportunities.</li>
<li>Develop and maintain relationships with authors within the legal community.</li>
<li>Identify emerging legal topics for potential publication.</li>
<li>Work with volunteer authors to develop books, practice manuals, and forms in various legal subject areas.</li>
</ul></li><li>Legal Publications Management<ul>
<li>Manage the end-to-end publishing lifecycle for treatises, practice manuals, and coursebooks. Identify potential delays early and intervene to keep projects on schedule.</li>
<li>Troubleshoot technical, editorial, or logistical obstacles independently to ensure projects stay on track.</li>
<li>Edit all submitted content and resolve queries with authors.</li>
<li>Coordinate and communicate with external vendors, including an offshore design team, a freelance indexer, and a third-party printer, to ensure the timeliness and quality of MSBA publications.</li>
<li>Ensure that all published content adheres to the association's style guide and legal publication guidelines.</li>
<li>Ensure quality control through final proof review.</li>
</ul></li><li>Research &amp; Legal Trends:<ul>
<li>Conduct research on developments in legal practice and ethics to identify relevant topics.</li>
<li>Review recent case law, statutory changes, and regulations that impact the legal profession for development into MSBA resources.</li>
</ul></li></ul><div>Intermittent/Adhoc Duties</div>
<ul><li>Regular attendance at high-profile section meetings, committee events, and networking mixers (typically 1&ndash;2 per month).</li>
<li>Participation in regular &ldquo;all-hands&rdquo; development days and strategic planning sessions at our Baltimore headquarters.</li>
<li>Support of the CLE team on-site for marquee educational programs, including the signature Legal Summit every June in Ocean City, Maryland, ensuring the smooth delivery of course materials and engaging directly with the members who use our resources.</li>
<li>To ensure organizational agility, you will regularly cross-train in other critical roles within the Learning and CLE departments to ensure seamless continuity of service.</li>

</ul><div>Qualifications &amp; Competencies</div>
<ul><li>Education and Experience:<ul>
<li>Juris Doctor (JD) degree from an accredited law school.</li>
<li>Active license to practice law in at least one U.S. jurisdiction.</li>
<li>Minimum of 5 years of legal experience.</li>
<li>Background in law journal leadership, judicial clerkships, or high-volume litigation/transactional practice preferred.</li>
</ul></li><li>Problem Solving &amp; Execution:<ul>
<li>Proven ability to identify and define workflow bottlenecks and proactively develop and implement the solutions required to break them.</li>
<li>A track record of navigating stalled projects by finding alternative solutions or adjusting production methods, ensuring all projects proceed to completion in a timely manner, regardless of obstacles.</li>

The Opportunity
We are seeking a Case Manager to support individuals as they move through different levels of care. In this role, you&rsquo;ll partner with clients and their care teams to coordinate services, develop discharge plans, and connect individuals with the community resources they need for ongoing recovery. You&rsquo;ll also play a hands-on role in monitoring daily progress, supporting treatment goals, and fostering smooth transitions across the system of care.

What You&rsquo;ll Do
<ul><li>Supports the delivery of care and engagement of individuals under care by participating in group supervision and by reviewing evaluations, assessments, treatment plans, and progress notes to understand the unique needs, strengths, preferences, and goals.
</li><li>Assesses the needs of the individual receiving care across multiple domains including mental health, medical, social, and other services by completing needs assessment and other appropriate tools.
</li><li>Partners with the client receiving care and their care team to identify clinical goals, interventions, and strategies, documenting in the discharge plan.
</li><li>Initiates and submits referrals to medical and Behavorial health care, community programs, and service providers.
</li><li>Develops and maintains strong working knowledge of care partners, service providers, and stakeholders across the system of care, maintaining a positive rapport.
</li><li>As approved and assigned, uses company vehicles to provide transportation for individuals entering, receiving, or discharged from care.
</li><li>Collaborates with care team members for admission and discharge from the residential milieu, engaging external stakeholders when appropriate and as directed during these transitions.
</li><li>Serves as a liaison as assigned and appropriate for internal and external stakeholders, care partners, and visitors.
</li><li>Demonstrates adherence to company policies, expectations, and behavioral competencies, both explicit and implicit.
