Principal Antineoplastic Stewardship Duties and Responsibilities<br>
1. Develops, enforces, and maintains policies and procedures that promote and enhance patient outcomes through safe,<br>
effective, cost-efficient, and appropriate medication therapy use for patients receiving antineoplastic therapy.<br>
2. Coordinates programs designed to minimize medication errors, adverse drug reactions and medication misuse with<br>
regards to antineoplastic therapy through improved reporting, analysis, and follow-up.<br>
3. Provides support to the institution&rsquo;s antineoplastic stewardship program, including:<br>
a. Overseeing daily/weekly/monthly prospective audit of antineoplastics<br>
b. Assisting in development and maintenance of an antineoplastic formulary approved by the medical staff<br>
c. Working with the Oncology/Hematology service to evaluate appropriate use of restricted antineoplastic<br>
d. Developing guidelines, policies, and other monitoring and intervention strategies for antineoplastic therapies<br>
e. Compiling data and metrics on antineoplastic use<br>
f. Working with the Oncology/Antineoplastic Subcommittee to maintain and update the network/institutional guidelines,<br>
policies, and protocols<br>
g. Reporting data and findings to the Oncology/Antineoplastic subcommittee and other stakeholders.<br>
h. Coordinating, facilitating, and promoting education, training, competency assessment and performance improvement of<br>

The MGH Department of Pharmacy is seeking a Clinical Pharmacist to join our Medication History/Transitions of Care team. This position will be focused on transitions of care, starting with admission medication histories, reconciliation, and supporting the continuum of care for patients from admission to discharge. The Clinical Pharmacist will supervise pharmacy technicians/students/residents participating in the daily conduct of this pharmacy program.

Clinical Pharmacists assume responsibility and accountability for the provision of safe, effective, and prompt medication therapy. Clinical Pharmacists proficiently provide direct patient-centered care and integrated pharmacy operational services with physicians, nurses, and other hospital personnel to deliver optimal medication therapy to patients with a broad range of disease states. These clinicians are aligned with target interdisciplinary programs and specialty services to deliver medication therapy management and to ensure pharmaceutical care programs are appropriately integrated throughout the institution. Pharmaceutical care services include, but are not limited to, assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. Clinical Pharmacists serve as a departmental resources and liaisons to other departments, hospital personnel, or external groups. They proficiently manage the appropriate use of medications according to professional and regulatory standards. They also conduct practice-related research and patient care, quality, and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice. Clinical Pharmacy Specialist - Emergency Department: Summary<br>
Responsible for the optimization of drug therapy by identification, resolution, and prevention of drug related problems and improving therapeutic outcomes. Provides patient care according to the laws and regulations governing pharmacists and the practice of pharmacy by the appropriate state and federal agencies. May be responsible for prescribing medications under specific collaborative drug therapy management agreements (CDTM).<br>
Works independently, and within the limits of established pharmacy/ambulatory care practice site's policies and procedures, as a member of the patient care team. May supervises support personnel during an assigned shift, operating within the limits of pharmacy policy and procedures, and the regulations of the Board of Registration Pharmacy.<br>

d. Maintain pharmaceutical care expertise through membership and participation in professional organizations, continuing education, and contributing to peer-reviewed literature.<br>

Under the direction of a staff pharmacist and/or pharmacy leadership team, follows established policies and procedures to perform a variety of routine support tasks that are integral to the operation of the pharmacy department.
