The MGH Department of Pharmacy is seeking a Clinical Pharmacist to join our Medication History/Transitions of Care team. This position will be focused on transitions of care, starting with admission medication histories, reconciliation, and supporting the continuum of care for patients from admission to discharge. The Clinical Pharmacist will supervise pharmacy technicians/students/residents participating in the daily conduct of this pharmacy program.

These clinicians are aligned with target interdisciplinary programs and specialty services to deliver medication therapy management and to ensure pharmaceutical care programs are appropriately integrated throughout the institution. Pharmaceutical care services include, but are not limited to, assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. Clinical Pharmacists serve as a departmental resources and liaisons to other departments, hospital personnel, or external groups.   The Alliance for Clinical Trials in Oncology Foundation (Foundation) is a foundation created to enhance and expand the ability of the Alliance for Clinical Trials in Oncology (Alliance) to conduct cancer clinical research and address important treatment questions through large-scale clinical trials. Through efforts of the Foundation in support of the Alliance, clinical trials and laboratory research are conducted to discover new or improved ways to prevent, treat, and cure many types of cancer, including leukemia and lymphoma, and cancers of the breast, prostate, lung, and gastrointestinal (GI) tract, and help educate the medical community on methods of cancer diagnosis, treatment, and prevention.

  In May 2014, the Foundation created the Alliance Foundation Trials, LLC (AFT), respectively, a wholly-owned subsidiary of the Foundation. 9. Maintains medication rooms by ensuring all appropriate medications are stocked in patient specific bins in the automated dispensing cabinets and by removing expired and discontinued medications.</p><p>
10. Empties return bins in the automated dispensing cabinets as part of the weekly schedule.</p><p>
11. Utilizes the unit-dose machine to package bulk medications and appropriately compounds non-sterile medications following standard formulas and procedures.</p><p>
12. Performs assigned cycle counts of the carousel on a regular basis, maintaining adequate records of completion.</p><p>
13. Takes accountability for medication management in their assigned patient care area.</p><p>
14. Assists in keeping the Pharmacy neat and clean by caring for equipment and cleaning after compounding prescriptions, manufacturing bulk medications, unit dosing, etc.
</p><p>15. Conduct temperature inspections of the refrigerator in the automated dispensing machine.</p><p>
16. Performs financial transaction activities which may include: Preparing patient billing information; billing third party prescription benefits; Troubleshoots insurance issues; and selects and uses pharmacy pay codes, payers and plans appropriate to each type of transaction.</p><p>
17. Inspects assigned medication storage areas monthly maintaining adequate records of inspection. All assigned areas must be inspected every month to meet standard.</p><p>
18. Greets and assists customers upon arrival to the pharmacy. Participates in point of sale cash transactions. Engages in customer services activities.</p><p>
19. Provides customer or patient assistance over the phone. Triage calls as appropriate.</p><p>
20. All assigned work is completed accurately by the end of each shift and all necessary quality control paperwork is completed accurately and with reasonable speed.</p><p>
