 The Health System Command Center Pharmacy Coordinator supports centralized pharmacy operations through a virtual command center model that serves multiple hospitals and ambulatory care settings across the health system.
 
 
 This role assists pharmacists and clinical care teams by performing medication reconciliation, patient outreach, medication access coordination, and prior authorization support while ensuring accurate medication management during care transitions.
 
 
 The coordinator also supports Hospital-at-Home programs, facilitating medication coordination, patient communication, and medication access services to ensure safe medication use outside the hospital environment.

Reporting to the Assistant Program Director, Transitional Housing the Clinical Coordinator, Transitional Housing supervises Case Managers and ensures the utilization of Harm Reduction and Housing First strategies and other best-practice clinical interventions to ensure the highest quality care for clients. Role Overview

HR Generalist Responsibilities:

Incentive &amp; Commission Relationship Management
<ul>
<li>Lead monthly/quarterly incentive confirmations directly with regional or business-unit leadership</li>
<li>Maintain and analyze regional performance reports to support incentive conversations</li>
<li>Own incentive tracking and reporting for assigned business lines</li></ul> 


Employee Relations &amp; Offboarding

<li>Conduct exit interviews as needed and identify themes to share with leadership</li>
<li>Partner with the Coordinator on exit processes, providing guidance on sensitive or non-routine separations=</li>
</ul>
Engagement &amp; Culture Programming
<ul>
<li>Help lead the quarterly company town hall for people shoutouts</li>
<li><ul><li>Coordinate new hire features, employee spotlights, and work anniversaries</li>
<li>Track and confirm details in advance (e.g., anniversary and spotlight nominations each quarter)</li>
</ul></li><li>Organize end-of-quarter celebratory recognition for teams/locations</li></ul> 


Projects &amp; Cross-Functional Support

 As the Recruitment Coordinator, you will be the face of the Talent Acquisition team based out of New York, supporting our domestic and international offices. You will have fantastic exposure to the various teams across the global business and will be one of the first points of contact for recruitment topics. You will take an active role in:

 Experience supporting a recruitment team by taking a lead role in the coordination of interviews and both internal / external meetings

 Experience closely interacting with candidates, hiring managers and recruitment agencies, providing high levels of customer service and consumer experience

 Proficient use of Outlook and Microsoft Office application packages, particularly Excel

 Experience working with an applicant tracking system, preferably Greenhouse
 </li>

 Experience in vendor systems

 The base pay for this role is expected to be between $80,000.00 and $110,000.00. The expected base pay range is based on information at the time this post was generated. This role may also be eligible for other forms of compensation such as a performance bonus and a competitive benefits package. Actual compensation for the successful candidate will be determined based on a variety of factors such as skills, qualifications, and experience.
