<br>The Jewish Board’s Community Behavioral Health treatment programs provide compassionate, high quality, evidence-based services to individuals and families in the communities we serve. Our staff use a culturally competent, person centered approach to help individuals and their families develop skills and resources to improve overall functioning, to instill hope, and to strengthen resiliency. Our programs work closely with community partners to address health disparities in our neighborhoods while also celebrating the strengths and resiliency of our communities.

PROGRAM DESCRIPTION
<br>The ACT programs provide an integrated set of evidenced based treatment, rehabilitation, case management and support services to individuals residing in NYC, in Shelter programs, and/or living in supportive housing programs or their own residences in the community who are diagnosed with severe mental illness and whose needs have not been well met by more traditional service delivery approaches. Through a mobile, multi-disciplinary team of staff, recipients are supported in their recovery through an individualized, person-centered model of care.
<br><br>
<b>POSITION OVERVIEW:</b><br>
This multi- faceted position works directly with adults living with severe and persistent mental illness (SPMI) and substance use disorders (SUD) living in NYC. In addition to providing direct care, this position provides administrative oversight and support to the treatment team to ensure the delivery of services is in accordance with regulatory and payer requirements. Clinical Coordinators use a recovery oriented approach to link recipients to the services they need to obtain housing and to achieve wellness self-management and increasing independence. Working in a team setting and primarily in the community, Clinical Coordinators assess risk and needs, develop person centered care plans, provide care management services, track and arrange appointments, educate clients and coordinate other aspects of clients’ health and community services. <b>KEY ESSENTIAL FUNCTIONS:</b><br>
<ul>
<li>Assume administrative tasks such as chart auditing, collaborating with outside partners, facilitating team meetings and being the point person in crisis situations when Program and Team Director are not available.</li>

<li>Assist with on-boarding/orientation of new staff</li>
<li>When needed/requested, lead team in-service training on topics that will improve service to clients.</li>

<li>Collaborative development of an Individualized Care Plan (ICP) with the client, the client’s family and/or caregivers in addition to other service providers, and review and revise ICP per regulatory standards or when there are significant changes in functioning or clinical status.</li>
<li>Provide care management services</li>
<li>Track all specialty medical, behavioral and support service referrals made for patient using Health Information Technology (HIT) provided.</li>
<li>Assure that client has access to, engages in and retains needed services as defined in the ICP.</li>

<b>EDUCATIONAL / TRAINING REQUIRED:</b><br>

Duties &amp; Responsibilities</b></h2><div><br>

</div><p><b>Purpose of Position</b>:</p><p>
This class of position may encompass supervisory, administrative and clinical activity with varying degrees of latitude for independent judgment involving day-to-day supervision of an operating segment of the Respiratory Therapy Department in the delivery of respiratory therapy services.
</p><p>May perform respiratory therapy procedures as required. All personnel perform related work.</p><p><b>
Examples of Typical Tasks</b>:</p><p>
Under general medical and technical direction of a duly licensed physician, with considerable latitude for independent initiative and judgement; may perform some of the duties described under Assignment Level I:</p><p>
1. Identifies quality assurance issues and ensures adherence to established protocols and standards of care.</p><p>
2. Provides in-service training related to the delivery of respiratory therapy services.</p><p>
3. May participate in research activities related to respiratory care.</p><p>
4. Participates in product evaluations.</p><p>
5. Provides clinical supervision in specialty care areas as assigned.</p><p>
Under medical and technical direction of a duly licensed Physician, with moderate latitude for independent initiative and judgment, performs the following functions and activities, in addition to or instead of duties described above for Assignment Level I:</p><p><b>
1.</b> Assists in the planning, development and coordination of the department quality assurance program.</p><p><b>
2.</b> Participates in the evaluation of discharged patients for home respiratory care.</p><p><b>
3.</b> Plans, develops and coordinates in-service training programs related to the delivery of respiratory therapy.</p><p><b>
4.</b> Plans, develops and coordinates research activities relating to respiratory therapy.</p><p><b>
5. </b>Plans, develops and coordinates product evaluations relating to respiratory care.</p><p><b>
6. </b>Arranges for the provision of clinical supervision in specialty care areas.</p><p><b>
7. </b>May be assigned duties and responsibilities of immediate supervisor in his or her absence on a temporary basis.</p>

Minimum Qualifications</b></h2><div><br>

</div><p>1. Graduation from an Associate Degree program in Respiratory Therapy approved by the New York State Board of Education or a program determined equivalent in accordance with the Commissioner of the State Board of Education's regulations; and,<br>
2. Possession of a current license to practice as a Respiratory Therapist in the State of New York; and,<br>
3. Possession of current registration as a Respiratory Therapist issued by the National Board of Respiratory Care.</p>
