Summary
 The Team Coordinator (TC) is responsible for overseeing and managing the operation of the clinical support team within the Clinical Support Program (CSP). The Team Coordinator assures the implementation of programs assigned effectively within the division. This position requires effective communication between the clinical support staff (consultants), Program Director, all program stakeholders, and other agency departments.
 
Essential Functions: 
<ul>
<li>Provide clinical supervision to the CSP clinicians to help them make comprehensive family system assessments integrating mental health, domestic violence and substance use issues</li>
<li>Provide administrative supervision to the CSP clinicians to maintain timely documentation and ensure compliance with standards set by The Child Center of NY, The Administration for Children’s Services and the State Office for Children and Family Services.</li>
<li>Provide leadership in team building and collaboration to enhance service delivery</li>
<li>Manage the clinical support team, implement and oversee the programs’ requirements as assigned. Such requirements include: assigned consultations, client and outside agency-related meetings, and trainings.</li>
<li>Provide leadership in building relationships within ACS field offices, TCCNY central office, preventive agencies, community based mental health, domestic violence and substance use service providers and hospitals</li>
<li>Build capacity of the child welfare workforce by developing/ conducting specialized trainings in mental health, domestic violence and substance use areas.</li>
<li>Build and maintain working relationships with providers city-wide</li>
<li>Develop and support employee growth for retention.</li>
<li>Communicate program aspects throughout the organization.</li>
<li>Oversee community/ client engagement, outreach, and stakeholder relationships.</li>
<li>Meet performance expectations/metrics, as defined for the position within the assigned program.</li>
<li>Participate in the following on an as-needed basis:</li>
<li>Conduct formal and informal consultations with ACS Child Protective Specialists on their cases which include mental health and/Intimate partner violence concerns. The CSP consultant will assist in assessing the presence of these concerns and help determine the specific level of interventions required to strengthen the family and reduce risk to the children, providing concrete tools and casework guidance.</li>
<li>Conduct cross consultations with their peers, and CASAC support specialists that involve substance use disorder, mental health concerns and intimate partner violence</li>
<li>Participate in family team meetings, case conferences and child safety conferences with ACS families to assist ACS workers with providing client with psychoeducation, referrals, and raising awareness of the concerns presented and impact on the children. Direct client contact is to model and/ or demonstrate effective interventions and engagement strategies for the benefit of caseworkers or other ACS staff.</li>
<li>Engage directly with families and/or support CPS Teams as they work with families to understand the pressing issues and pertinent history, and to identify and assess areas of concern and strength</li>
<li>Conduct assessments, recommend effective interventions and engagement strategies for the benefit of caseworkers, other ACS staff and families.</li>
<li>Make appropriate referrals for mental health, domestic violence and substance use services.</li>
<li>Attend case reviews as an important training tool, as per request of ACS and approval of the Program Director.</li>
<li>Attend CPS citywide mental health, IPV and substance use meetings- as determined by the Program Director.</li>
<li>Conduct occasional community/ field Visits-including but not limited to home visits, schools, hospitals (i.e. discharge meetings) and other treatment provider sites to assist with engaging clients and partnering with ACS.</li>
<li>Perform other related duties as assigned by the Program Director.</li>

Supervisory Responsibilities: 
<ul>
<li>Manages the overall direction, coordination, and evaluation of the clinical support team. Carries out supervisory responsibilities in accordance with agency policies and applicable laws. Solid comprehension of Collective Bargaining Agreement (CBA), where applicable.</li>
<li>Responsibilities include interviewing, hiring, planning, assigning, directing work, leading trainings, rewarding, disciplining employees, performance reviews, and addressing complaints and resolving problems.</li>

Administrative Responsibilities: 
<ul>
<li>Ensure timely preparation of administrative reports for central administration of TCCNY and ACS.</li>
<li>Schedule and manage the clinical support team’s schedule regarding consultations, related field work (i.e. community visits, family meetings/ conferences) and other meetings/ trainings)</li>
<li>Manage and schedule ACS support program requests in a timely manner</li>
<li>Aid in coordinating staff acquisition, interviews, orientation, training, payroll and evaluations with the Program Director and Human Resources.</li>

Position Summary:
This position is responsible for obtaining and logging case management (CM) assessments/reassessments and HIV Risk assessments for a targeted cohort of Amida Care&rsquo;s HIV-Negative, Transgender and Non-binary (TGNB) membership. This position will also be responsible for conducting essential case management responsibilities for this portion of the membership, including TGNB health navigation, HIV prevention health education, and referrals to community partners to address essential social determinants of health. Additionally, this position will work with external case management agencies and care teams to collaborate on case management needs and to obtain and record necessary documentation. and progress notes received from external case management agencies, by educating sites and collecting documentation.
Responsibilities:<ul><li>
Conduct telephonic reassessments and HIV risk assessments every 180 days for assigned caseload.</li><li>
Refer members to the appropriate case management agency after assessment completion, if requested by member.</li><li>
Perform monthly generation and on-going review of case management site rosters and identify members who are actively receiving case management.</li><li>
Obtain comprehensive psychosocial case management assessments/progress notes for those active members assigned to a case management agency.</li><li>
Document elements of the assessment, including the needs identified and member&rsquo;s input.</li><li>
Review and address Gaps in Care with members and providers as applicable.</li><li>
Refer members deemed unreachable to the Retention in Care Unit (RICU) and external agencies as requested through Unite Us and perform referral follow-up.</li><li>

Breaking Ground operates nearly 4,000 units of housing across New York City, along with housing in upstate New York and Connecticut. Supportive housing - affordable housing paired with services designed to help people maintain their homes for the long-term - is widely recognized as a proven and cost-effective solution to chronic homelessness.
 
But we didn't stop at providing housing. Breaking Ground's programs and services help people experiencing street homelessness - especially those who have been on the streets the longest - to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home.
 
Reporting to the Assistant Director, Programs, Safe Options Support the Clinical Coordinator, Safe Options Support (SOS) supervises Case Managers and Consumer Advocates for the SOS team serving Staten Island. The Clinical Coordinator also manages a small caseload. The Clinical Coordinator ensures the utilization of Harm Reduction and Housing First strategies and other best-practice clinical interventions to ensure the highest quality care for clients. The Clinical Coordinator is responsible for ensuring client records are kept in compliance with regulatory and agency standards.
 
The Clinical Coordinator works primarily during business hours Monday-Friday, with off hours availability in case of emergency or for special projects/initiatives.
 
ESSENTIAL DUTIES AND RESPONSIBILITIES: 
<ul>
<li>Supervise a team of Case Managers and Consumer Advocates</li>
<li>Provide regular, structured supervision meetings for direct reports</li>
<li>Promote effective strategies for case management following Housing First, Harm Reduction, Crisis Prevention Institute (CPI), Motivational Interviewing and trauma-informed care models of practice</li>
<li>Develop and implement individual service plans; ensure documentation of client contact and progress is comprehensive and timely</li>
<li>Interview and assess potential clients</li>
<li>Provide clinical direction and leadership to the program, help troubleshoot, problem solve, and strategize solutions around client engagement and challenging behaviors</li>
<li>Promote a staff and resident culture that emphasizes housing placement as a measurement of success; ensure that clients are ready for housing placement</li>
<li>Oversee development and implementation of individual service plans; ensure documentation of client contact and progress is comprehensive and timely</li>
<li>Assess and evaluate client functioning</li>
<li>Supervise and conduct case conferences</li>
<li>Coordinate delivery of care with multiple service providers, particularly outreach teams and transitional and permanent housing providers</li>
<li>Provide support and guidance to front line staff in managing difficult situations and successfully de-escalating conflict</li>
<li>Perform other duties as assigned</li>
</ul>
MINIMUM QUALIFICATIONS: 
<ul>
<li>NYS Licensed Masters Social Worker (LMSW) required. Must obtain LMSW within first six months of employment</li>
<li>Minimum 3 years of experience with related populations; previous supervisory and administrative experience preferred</li>
<li>Thorough clinical understanding of relevant service delivery concepts and structures including Housing First, client centered motivational interviewing, Harm Reduction models, behavioral and medical health systems, and the ability to access and negotiate the full range of services for recipients</li>
<li>Experience with crisis intervention, including, risk assessments and incident management</li>
<li>Ability to work successfully with a wide range of internal and external stakeholders</li>
<li>Demonstrated success in working in a fast-paced environment with multiple priorities</li>
<li>Ability to delegate and motivate staff to achieve deliverables</li>
<li>Ability to communicate (verbally and written) with diverse populations and stakeholders</li>
<li>Valid NY, NJ, CT, or PA driver’s license with a good driving record, as defined by Breaking Ground, is required</li>
<li>Proficiency with Microsoft Office Suite programs including Word, Excel, Outlook, and Microsoft Teams; familiarity with Access-based databases and the ability to learn new programs are required</li>
